The personalised service aims to provide individualized support and guidance to senior clients to create action plans towards improving and sustaining health and quality of life and facilitate connections to community resources based on client’s needs and interests

The service includes exercise prescription, lifestyle modification support, and social prescribing.
Examples of non-clinical community support services:

Lifestyle advice for chronic disease management or weight management
Healthy living – Exercise, physical activity, diet and nutrition, activity engagement
Connection to community resources to build social networks to reduce risk of isolation and loneliness
Supporting independence in community living
Home adaptation advice for seniors
Caregiver education and support

TWB Programmes and Group Workshops

Programmes and workshops are suitable for seniors who are 50 years old and above, can converse in basic English, Mandarin, or Malay, and are medically stable.
